Accurate detection genetic and epigenetic changes are critical for understanding fundamental biology, how it goes wrong in disease and for translation into better diagnostic tests and interventions. The duet platform uses a hairpin approach to provide simultaneous, high accuracy genetics and epigenetics from a single low input sample, including the ability to distinguish activating 5hmC from suppressing 5mC.

The performance of duet in liquid biopsy settings has been improved through the addition of single stranded ligation to the workflow in the duet mosaic products. Single stranded ligation improves the capture of shorter disease-derived cfDNA fragments and improves analytical performance across genetics, methylation and fragmentomics in this challenging sample type. The duet mosaic workflow also eliminates hypomethylation artifacts that would otherwise confound deconvolution and tissue of origin (ToO) analyses of cfDNA through the elimination of undesirable effects of end repair and A-tailing.

The 6-base genome, where 5mC is distinguished from 5hmC, reveals biomarkers that are hidden by traditional methylation readouts which conflate 5mC and 5hmC into a modC signal. Beyond revealing novel biomarkers, 6-base data can be used to more confidently infer gene expression across tissue and liquid biopsy applications. In tissue the analysis identified tissue-specific expression programmes and in cfDNA it enabled the understanding of tumour resistance mechanisms in prostate cancer, suggestive of novel therapeutic avenues.

In summary, the duet platform provides complete genetics whilst simultaneously providing complete epigenetic and fragmentomic information from a single low input sample. This reveals novel biomarkers, can be used to understand underlying biology and creates new opportunities for integrated biomarker discovery and more sensitive ctDNA detection.
